The Diabetes Self-Management Program, is a 6-week program, given once a week for 1 1/2 hour to 2 hours per week. This program is free and will be conducted in English. Once a participant completes 4 out of 6 sessions, the participant will receive a certificate of completion and a text book called "Living Healthy with Chronic Conditions."
Workshops are facilitated by certified Community Health Educators who work in Brooklyn’s neighborhoods everyday, and teach classes in both English and Spanish.
Topics covered include: 1) diabetes risk factors; 2) techniques to deal with common symptoms & problem solving strategies; 3) self monitoring and use of glucose meter; 3) physical activity; 4) healthy eating; 5) appropriate use of medications; 6) communicating more effectively with health care providers and involving family and friends in diabetes care.
The Diabetes Self-Management Program is open to those with pre-diabetes, diabetes, those at risk for developing diabetes and/or caring for someone with diabetes.
