Lourdes Hospital Diabetes Prevention Project

One in three adults have pre-diabetes and 90% of adults with pre-diabetes don't know they have it. Lourdes Hospital is helping people decrease the risk of developing diabetes.

The diagnosis of pre-diabetes means that although your blood sugar is higher than normal, it is not high enough to be diagnosed as diabetes. People with pre-diabetes are at significantly increased risk of developing diabetes in the next 5 years. The Lourdes Diabetes Prevention Project staff works with patients and their primary care provider to help them make the changes needed to decrease the risk of developing diabetes. Services provided include information about community resources that may be helpful, support groups, walking groups, access to a health coach for one-on-one advice for making lifestyle changes, help with referrals to specialty services, when needed, available to answer questions that may come up, monitor a patients progress and provide regular reports to the patient and primary care provider, provide information updates about pre-diabetes and strategies that may help the patient make the lifestyle changes needed to decrease the risk of developing diabetes, help setting goals for health and provide a free 1-on-1 meeting with a health coach.
